Antibiotic prescription log

We have a little book and we write in it when we give out antibiotics - it is kept in the locked cupboard with the antibiotics and never leaves it. We record - date, patient name, name of medicine, expiry date and the lot number. Out book has been going for years! Not sure how long you have to keep records for.

Hi, at my practice we issue antibiotics. I was wondering how they were logged and how long they were kept for?
