Appraisal

Hi got new appraisal form from work, need to fill it in at home and return it when i have the appraisal, the problem is it seems to be writing about how great you are and what a good job you do. I find this very uncomfortable, I hope that i am a good nurse and do the job to the best of my abilities and would hope that the managers already see this. Any suggestions welcome thanks.
