Dental Nurses Recognised in Their Role in Prevention

fluoride application course dental nurses 24From April 2026, NHS dentistry in England is set to undergo a series of reforms. Among these wide-ranging contract changes is a development that stands out for the dental nursing profession.

For the first time, a preventative intervention delivered by a suitably trained, indemnified and registered dental nurse will generate its own Units of Dental Activity (UDAs). A new 0.5 UDA course of treatment will be introduced, allowing fluoride varnish application to be provided to children between regular dental examinations. Although this fee is low, it is a start in the right direction. 

What is changing?

Under the reformed NHS dental contract:

  • Fluoride varnish can be delivered as a standalone course of treatment in between examinations.
  • It will attract 0.5 UDAs.
  • It can be provided by appropriately trained and indemnified dental nurses.

Dental nurses will still work under a dentist’s prescription or as part of a structured dental health programme, ensuring all care remains strictly within the scope of practice set by the General Dental Council.

Why this matters

For years, dental nurses have been involved in prevention by delivering oral health education, supporting behaviour change, and applying fluoride varnish. Yet, much of this vital work has been unrecognised within the UDA framework.

This reform changes that. By creating a fluoride-only course of treatment, the contract finally acknowledges that dental nurses are skilled clinicians within the dental team. It validates our essential role in maintaining long-term oral health.

A shift towards prevention

The wider contract reforms emphasise urgent care, complex care pathways, and quality improvement. Prevention now sits firmly within that structure.

Applying fluoride varnish at the correct 3- or 6-month intervals is evidence-based and reduces caries risk. Allowing these appointments to occur between examinations means:

  • Children can access preventative care at the clinically right time.
  • Practices can design nurse-led preventative clinics.
  • The dental team can work more collaboratively to support patient stability.

Appropriate Training

Registered dental nurses should only provide fluoride varnish treatment if they are properly trained, indemnified, confident, and competent to do so.

Fluoride varnish is a prescription-only medicine. As regulated professionals, dental nurses must be able to evidence appropriate training and maintain clear records should any concerns arise. This protects patients, practitioners, and practices.

Risk Awareness

Nurses must understand indications, contraindications, correct dosage, and safe handling. As a prescription-only medicine, fluoride requires awareness of medical history, allergies, ingestion risk, and emergency protocols.

Delivering fluoride varnish, particularly to children, requires behaviour management, reassurance, and the ability to manage anxious patients safely. Nurses must be confident leading the appointment while remaining within scope and under prescription.

Communication and Knowledge Confidence

Fluoride is an area that attracts questions and, at times, misinformation. Dental nurses need a solid understanding of the evidence base, safety profile, and public health guidance in order to respond professionally and confidently to parental concerns.

Oral Health Education and Prevention Strategy

Application should sit within a broader preventative plan. Nurses must be able to reinforce dietary advice, brushing techniques, and recall intervals to support long-term caries reduction.

Recognition within the NHS contract creates opportunity. Proper training ensures that opportunity is delivered with clinical integrity, regulatory compliance, and professional confidence.
